Role Overview
The Discharge Planner is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care.
Core Responsibilities
- Participate as an integral member of the treatment team including attendance at program staff meetings, treatment planning meetings, and assigned committees.
- Work closely with therapists, utilization review coordinator, and providers in liaison with insurance companies to coordinate necessary documentation and discharge planning to promote quality management of treatment items across the continuum of care.
- Assists with chart audits and completion of medical records.
- Provide assistance and work closely with the treatment team to identify the needs of the patient and take appropriate action to see that the needs of the patients are being met.
- Work closely with the treatment team and families to encourage involvement in the treatment of the patient.
- Facilitate communication with referral sources and mental health care professionals in the treatment planning process to include discharge planning, aftercare plans, and arrangements for the patient.
- Work closely with community agencies to ensure that the needs of the patients are met.
Qualifications
- Education: Bachelor's degree from an accredited college or university in social work, psychology, or a clinical related mental health field preferred.
- Experience: Considerable knowledge of the principles of clinical treatment and social casework strongly preferred. 1+ years working in an inpatient mental health setting, or comparable experience strongly preferred.
- Additional Requirements: Successful completion of BLS and Handle with Care Training within 30 days of employment. May be required to work flexible hours and overtime.

- Monday - Friday, 8:00am - 4:30pm
