Discharge Planner

Perimeter Behavioral of Jackson has an immediate opening for Discharge Planner. We are seeking individuals who are committed to improving the lives of those who experience mental health problems.
Perimeter Behavioral of Jackson is one of the premier behavioral health hospitals providing inpatient youth and adolescent treatment. The Discharge Planner's responsibilities include the following:

• Assist the treatment team in the preparation of the initial treatment plan and the master treatment plan, to include, working with the parents, families and others to encourage active involvement in treatment of the patient.
• Work closely with the Utilization Review Coordinator, doctors and therapists in liaison with insurance companies to coordinate necessary documentation and discharge planning.
• Participate as an integral member of the treatment team, including attendance at program staff meetings, treatment planning meetings and assigned committees.
• Communicate with referral sources or other mental health professionals on progress in treatment of patient and, where appropriate, involves referral sources in treatment planning process to include discharge planning and aftercare plans and arrangements and arrange distant family therapy as appropriate.
• Coordinate with the other members of the treatment team to arrange appropriate aftercare referrals and communicate aftercare planning to patients, families, and community resources
• Follow policies and procedures regarding patient rights, confidentiality, and privacy.
• Demonstrate understanding and respect for cultural and religious beliefs of people served.
• Demonstrate the ability to recognize and report suspected abuse and neglect according to policy, procedure, and law.
   

Qualified candidates for Discharge Planner should have the following:. 

• A Bachelor's Degree in social work, psychology or related area, and two (2) years’ experience in an inpatient or outpatient mental health setting, and/or comparable experience preferred. 
• Additional Requirements: Valid State Driver’s License. Able to successfully complete CPR and Crisis Prevention Intervention Training during New Hire Orientation.
