Position Summary
As a PRN Case Manager, you will provide case management services that support the treatment, recovery, and successful discharge of adolescent patients. Working alongside therapists, physicians, nurses, and residential staff, you will help coordinate care, communicate with families and community providers, and advocate for each patient's individual needs.

Qualifications
- Bachelor's degree in a Human Services or Social Services field
- Must be at least 21 years of age
- Knowledge of clinical treatment and social casework principles
- Understanding of child and adolescent development
- Experience working with children or adolescents with emotional and behavioral challenges preferred
- Strong communication, organization, and documentation skills
Responsibilities
Care Coordination
- Collaborate with therapists, physicians, nurses, and residential staff to coordinate patient care
- Participate in treatment team meetings and discharge planning
- Coordinate therapeutic passes and related documentation
- Maintain accurate, timely, and confidential patient records
- Support quality improvement initiatives
Family & Community Collaboration
- Communicate with families regarding treatment progress and therapeutic passes
- Coordinate with referral sources, schools, and community providers
- Provide education regarding treatment plans and discharge recommendations
- Ensure confidentiality in accordance with HIPAA and facility policies
Patient Advocacy
- Identify and address patients' social, educational, family, and discharge needs
- Demonstrate sensitivity to cultural, developmental, and individual differences
- Recognize and report suspected abuse or neglect
- Assist patients and families in understanding treatment processes
Professional Responsibilities
- Promote teamwork and positive collaboration
- Follow organizational policies, ethical standards, and safety procedures
- Complete assigned documentation accurately and on time
- Perform other duties as assigned to support patient care
Schedule
- PRN (As Needed)
- Weekday availability preferred
- Hours will vary based on patient census and department needs
- In-person position located in Springdale, Arkansas
Compensation
Competitive hourly pay based on education and experience.
Reporting Relationship
- Reports to the Director of Clinical Services
- No supervisory responsibilities
Work Location
This is an in-person position located in Springdale, Arkansas. Candidates must be able to reliably commute to the facility.
